COSSMA Inc., which I’m honored to direct, is a non-profit organization with 22 years of existence offering heath care services to the communities in the central and eastern areas of Puerto Rico.  We have clinics conveniently located in Cidra (Central Office), Humacao, Yabucoa, San Lorenzo and Las Piedras.  Between all the clinics we have approximately 27,000 patients.  In addition, we have a shelter, Hogar Rayos de Esperanza for homeless men with HIV in the municipality of Juncos and soon adjacent to the shelter, we will be inaugurating the community of Esperanza Village (nine transitional single family homes) for persons with HIV/AIDS.

Thanks to the effort and dedication of our workforce committed to the health care of the community served, they carryout their work in an effective, responsible and dedicated manner.  COSSMA is accredited by the “Joint Commission”, this accreditation is considered the Gold Standard in health care.  Our dental clinics are also accredited by the American Dental Association.

We are also a 501 c (3) tax exempt organization certified as a non-profit.  This certification has opened opportunities to acquire additional state, federal, foundation and local funds to provide excellent services with the best resources available.

We have a firm commitment with our patients and clients and we live it every day as expressed in our mission statement, “To improve the quality of life of the community by providing excellent comprehensive primary health care services”.  We continue to search for excellence and patient satisfaction by continuously being vigilant in recognizing their needs and the community needs.  For this reason we empower community leaders to serve as peer mentors in their communities and thus better reaching the communities.

All our staff is highly qualified, including the medical staff with physicians with specialties in Family Medicine, General Medicine, Obstetric/Gynecologists, Pediatricians and Dentist.

We specialize in prenatal and post partum care, in control of diabetes, hypertension, asthma, children and adolescent care, elderly care and behavioral health.  You can choose your primary care physician amongst the excellent committed health care providers of COSSMA.  We also offer dental, laboratory and pharmacy services.

We have available support services with the nutritionists, social worker, health educator and mental health providers that complement the services provided by the physician.  COSSMA provides comprehensive primary health care services to all the residents, no matter what their capacity of payment is.  We provide reduced prices based on income and family size.  Our strategy to fulfill our mission is based on:

• Maintaining attractive facilities, community located with after hour coverage including Saturday’s for filling the needs of the community.
